Here’s what’s actually changing in Pennsylvania, who’s affected, and what to do now so nobody loses coverage over paperwork.

The three changes that matter

1. Eligibility checks every 6 months (starts December 31, 2026).
Many adults on Medicaid will have their eligibility reviewed twice a year instead of once. Miss a renewal packet, and coverage can lapse — even if you still qualify.

2. Work requirements (start January 1, 2027).
Adults aged 19–64 who got Medicaid through the “expansion” category will need to show 80 hours per month of work, job training, school, or community service — or qualify for an exemption.

3. Less retroactive coverage (starts January 2027).
Today, Medicaid can pay bills going back up to three months before you applied. That window is shrinking — so applying quickly after a lapse matters more than ever.

Who does NOT need to panic

Most seniors and people receiving long-term care services are exempt from the work requirements. That includes people who are:

65 or older, or on Medicare as well as Medicaid

Receiving long-term services through Community HealthChoices (CHC)

Medically frail, disabled, or pregnant

Caring for a child under 13 — or caring for a disabled family member

That last one surprises people: family caregivers are generally exempt — but you may need to prove it. Which brings us to…

What to do now (before the letters arrive)

Update your contact info with the County Assistance Office (or on COMPASS, Pennsylvania’s benefits portal). Most coverage losses start with a letter that never arrived.

Watch the mail from September 2026 onward. DHS begins sending notices about the new rules in fall 2026. Don’t ignore anything from DHS — deadlines are short.

Build a “renewal folder” today: photo ID, proof of income, proof of disability or caregiving responsibilities, last year’s renewal paperwork. When the packet comes, you respond in days, not weeks.

If you’re a caregiver, document it. A doctor’s note about the person you care for, or your role in their care plan, can support your exemption.

Know where free help lives. The Pennsylvania Health Law Project offers free help with Medicaid problems and appeals. Your local Area Agency on Aging can help too.

What happens if coverage lapses anyway?

Coverage lapses in 2027 will often be procedural — a missed form, not actual ineligibility. If it happens: re-apply immediately (the retroactive window is short now), ask for help, and don’t quietly stop services you depend on.

At Rittenhouse Home Care, we’ve built for this. Every client family gets renewal-readiness support — we help you keep documents staged, flag redetermination dates, and if coverage ever lapses mid-care, our Coverage Bridge program keeps care going at a reduced rate while we help you get reinstated. Care shouldn’t stop because paperwork did.

Have questions about a parent’s coverage and care?
